Progress Report Forms

SFI program participants should select ONLY ONE form from the appropriate category (certified or non-certified) in Section I below. Only one completed form per organization or company will be accepted. Please combine all of your organization’s facilities and land-based operations (if applicable) into one form before submitting it to the SFI, Inc. Sections II and III contain supporting materials and references to aid you in completing the forms.

SFI, Inc. staff can send you a hard copy of the form if you are unable to download the PDF version. Contact Rachel Dierolf.

Return the completed form to Rachel Dierolf at SFI, Inc. by March 31, 2008.

I. SFI Report Forms
Chose one form from list “A” or “B” below:

A. SFI Program Participants that have chosen not to third party certify their operations

  • Public Agency, Conservation Group and Other Non-Industrial Forest Landowners (PDF or Word)
  • Industrial Forest Landowner Only (PDF or Word)
  • Industrial Forest Landowner With Manufacturing Facilities (PDF or Word)
  • Primary Manufacturing Facilities (no forestland) (PDF or Word) – More than 5% of the furnish is roundwood (logs or pulpwood) and wood chips of field manufactured-primary-and mill-residual origin
  • Recyclers and Other Secondary Manufacturing Facilities (no forestland) (PDF or Word) – More than 95% of the furnish is sourced from secondary sources: semi-finished solid wood, paper, market pulp, recovered wood fiber (recovered paper) or composite products obtained from a primary and or secondary producer, agricultural sources, and sawdust or dry shavings produced as a by-product of a primary or secondary manufacturing process.
  • Universities and Colleges (PDF or Word)

B. SFI Program Participants that have completed or are in the process of completing third party certification

  • Certified: Public Agency, Conservation Group and Other Non-Industrial Forest Landowners (PDF or Word)
  • Certified: Industrial Forest Landowner Only (PDF or Word)
  • Certified: Industrial Forest Landowner With Manufacturing Facilities (PDF or Word)
  • Certified: Primary Manufacturing Facilities (no forestland) (PDF or Word) – More than 5% of the furnish is roundwood (logs or pulpwood) and wood chips of field manufactured-,primary-and mill-residual origin
  • Certified: Recyclers and Other Secondary Manufacturing Facilities (no forestland) (PDF or Word) – More than 95% of the furnish is sourced from agricultural sources, recovered wood fiber, recovered paper, or sawdust and dry shavings produced as a by-product of a primary or secondary manufacturing process
  • Certified: Universities and Colleges (PDF or Word)
